Tri-State Generic Plan/Tri-State RX Plan SPD
IMPORTANT
NOTICE: This is the Summary Plan Description (“SPD”)
of the plan of benefits (“the Plan”) of the Building
Service 32BJ Health Fund (“the Fund”) with regard
to the Tri-State Generic/Tri-State RX Plan. Your rights to
benefits can only be determined by this SPD, as interpreted
by official action of the Board of Trustees (“the Board”).
You should refer to this booklet when you need information
about your Plan benefits. In addition, the Board reserves
the right, in its sole and absolute discretion, to amend the
Plan at any time.

Is this your plan?
Your health benefits are determined by the collective bargaining
agreement or contract negotiated between your union, SEIU Local
32BJ, and your employer. In general, subject to other requirements
such as number of days or hours worked, the following groups of
members are covered by the Tri-State Generic/Tri-State RX:
The Tri-State Generic/Tri-State RX plan covers part-time
members working for cleaning
contractors in:
- New Jersey
- Westchester and Hudson Valley
- Long Island
- Fairfield County, CT
There are also a number of other plans offered by the 32BJ Health
Fund, such as Tri-State or Tri-State Preferred, and Metropolitan.
